To Become A Patient

After collecting some basic information, we can schedule a consultative appointment with Dr. Poulos at our office. Dr. Poulos is in high demand and tends to book several weeks in advance- something to keep in mind!
The Consultation with Dr. Poulos
Consultations with Dr. Poulos cost $150. Should the patient decide to go with Dr. Poulos as their cosmetic practitioner, this $150 fee will be deducted from the total cost of the treatment and is automatically factored into the proposal.
Patients should be advised of our cancellation policy stipulating that all new clients must provide their credit card information to schedule the initial consultation. All patient credit card information is kept secure at all times. The patient’s card will only be charged if they cancel their appointment with less than 24 hours' notice. Arriving excessively late to the appointment or missing it altogether may also result in their card being charged. If a patient fails to notify us and come in, their card will be billed a $150 “No-Show” fee.
Completing the Forms
Before their appointment, patients will receive a link to their email addresses for online forms regarding their medical history. Patients who check in at our office will be asked to complete HIPAA, financial, and photographic consent documents.
On the Day of the Treatment
Upon arrival to the consultative appointment, patients must complete extensive paperwork regarding their demographics, medical history, and goals to improve their appearance. While non-invasive procedures are typically not covered by insurance plans, we will ask to photocopy the patient’s insurance card to keep on file.
A team member will then lead the patient into an exam room, where they will change into a robe and meet with Dr. Poulos. Once the patient and Dr. Poulos agree on a definitive treatment plan, Dr. Poulos will give them a proposal outlining all the included fees for their treatment.
Patients must make a 10% non-refundable deposit for booking and administrative fees when planning their treatment. They will pay the remaining balance on the day of the treatment (or at another time determined by our office). Dr. Poulos accepts various forms of payment, including cash, check, bank check, VISA, MasterCard, Discover or AmericanExpress credit cards, or financing through Care Credit.
